We are looking for adolescents between the ages of 12-17 years old with Diabetes, Polyarticular Juvenile Idiopathic Arthritis (JIA), Juvenile Psoriatic Arthritis (JPsA), Plaque Psoriasis (PsO) or another chronic medical condition to participate in our upcoming study. If you have a child that may be interested, please submit the following questionnaire for them to be considered for a study that pays $250 to your child and $100 to you!
